Community Participation Services

Community Participation Supports and Services (CPS) help people engage in meaningful community experiences through personalized goal setting based on their interests. CPS offers social, recreational, virtual, employment, and volunteer opportunities designed to build skills, promote independence, and encourage connection with others.

Activities are delivered in small groups and tailored to different support needs. Focus areas include

  • Life skills (e.g., cooking, budgeting, personal care)
  • Communication and relationship-building
  • Community engagement, events, outings, and travel
  • Recreation, health, and well-being
  • Self-advocacy

When not out in the community, participants can take part in in-house options such as:

  • Arts, music, and games
  • Fitness, literacy, health, and education
  • Cooking and meal prep
  • Planning special events
  • Relaxation and mindfulness activities

Some offerings may include fees. Direct Support Professionals assist individuals in building relationships and exploring new interests in both in-house and community settings.

Hopkins Community Participation Services

Hours of Operation: Monday to Friday, 8:00 a.m. – 4:00 p.m.

Hopkins Community Participation Services offers a wide range of life‑skills programs and meaningful activities for adults attending anywhere from 1 to 5 days per week. Our team of dedicated professionals supports participants with life‑skill development, goal planning, and community involvement.

The program features a unique layout consisting of four separate program areas, each designed to promote learning, connection, independence, and engagement.

Our activity calendars include a variety of options such as:

  • Individual goal planning
  • Building community connections and relationships
  • Volunteer opportunities
  • Exercise and wellness programs
  • Baking and cooking activities
  • Music therapy
  • Recreation and leisure programming

This environment provides participants with a supportive, person-directed experience that encourages growth, skill building, and community participation.

Adult Case Management is a collaborative and personalized person directed process based on your goals, needs, and values. It focuses on planning, advocacy, and connecting you with community resources to help you reach meaningful outcomes.

The Adult Case Management Coordinator (ACMC) will work with you to focus on the goals that matter most to you and involve the people important to you. Your ACMC will coordinate meetings with other service providers and support you during the meetings. They will also help with problem-solving and connect you to community resources such as education programs, social activities, job opportunities, support groups, financial aid, housing, transportation, and specialized services like counseling. Your relationship with your case manager is based on your active involvement in the planning process. The case manager facilitates goal-oriented planning with clearly defined supports, ensuring that you and your family are always involved in the decision-making.

An Adult Case Management Coordinator (ACMC) will work closely with you and the people who matter most in your life. Together, you'll identify priorities and create a plan that supports your goals. Your ACMC will:
